Case Management & Home Care Services To help elders remain independent and in their own homes, our case managers meet with them to assess their needs, determine eligibility for services, and develop a care plan. This is done during a home visit which takes approximately an hour. The Case Manager gathers information concerning an applicant’s health, medications, tasks the elder needs assistance completing, income and any other concerns that may arise during the visit. We encourage caregivers to participate in the assessment and planning of services. Case managers authorize services based on the individual needs of each elder or refer to other community agencies. Click here to go to a list of services offered by Elder Services of Worcester. Once a care plan is developed that best meets an elder’s needs, case managers review and monitor it on a regular basis.
